Quarterly Treatment Plan Reviews
Quarterly treatment plan reviews are designed to ensure a personalized and flexible approach to resident care. Our process is tailored to each facility’s unique needs, ranging from formal monthly or quarterly review meetings to informal consultations with staff during facility visits or as needed. In these reviews, our interdisciplinary team collaborates closely with your facility to assess pharmacy reports, treatment plans, medications, and behavioral issues. When appropriate, we provide Gradual Dose Reductions (GDR) to optimize residents’ care. This customized approach guarantees that every resident receives the best care suited to their individual requirements.
Pharmacy Reviews
Our pharmacy reviews address medication adjustments, polypharmacy, medication duplication, and reductions. We maintain thorough survey-driven documentation to ensure the highest standard of care.
Gradual Dose Reduction Tracking
We meticulously track and report all reductions, whether successful or not, to prevent survey citations and F-Tag’s. We also provide non-pharmacological interventions during times of gradual dose reductions.
Appropriate referrals:
- Our services are ideal for residents currently on Antipsychotic, Antidepressant, Antianxiety, Mood Stabilizers, Stimulants, Hypnotic, or any psychotropic medication. We specialize in addressing mood problems such as depression, anxiety, withdrawal, delusions, hallucinations, and paranoia, as well as behavior problems, including sexually inappropriate behavior, physical/verbal aggression, wandering, and hoarding. We also provide support for individuals experiencing suicidal thoughts or statements and a history of trauma.
Diagnostic Assessment
Our services begin with a comprehensive diagnostic assessment. This includes an initial evaluation, diagnosis, and the formulation of a treatment plan with objective, outcome-based goals.
Individual, Group and Family Psychotherapy
We offer measurement-based psychological services tailored to achieve the outcome-based goals established in the treatment plan. Our services cover individual, group, and family therapy sessions to meet diverse needs.
Plan of Care Review
To ensure continuous progress, we conduct quarterly reviews to assess how residents are advancing toward their goals and to evaluate their current symptoms and functioning. These reviews guide us in setting new objectives or planning for potential discharge.
Psychological Testing
Our neuropsychological and psychological evaluation services assist with differential diagnosis, treatment, and discharge planning.
Discharge Planning
Upon completion of our services, we provide recommendations to promote ongoing stability and the maintenance of functioning for residents.
